Care Data Sharing with Health & Social Care Information Centre

Every household in the UK will be receiving information regarding this and it has been widely publicised in the media. From April GPs will be obliged to share annoymised data with the HSIC via electronic transfer for research and analysis purposes. Unless patients "opt out"" of the scheme they will automatically be included. (please note if you previously opted out of the summary care records this will not cover you).

There are 2 levels of opt out, firstly where the data does not leave the GP surgery and secondly where the HSIC will not be able to transfer your data to a third party. For information go to www.hscic.gov.uk

opt out form REQUEST_FOR_WITHHOLD_OF_PERSONAL_CARE_DATA.doc if you would like to opt out of this scheme please complete, sign and return this form to the surgery.
